On the death of the head of the family / main earning member of family, financial support is provided to the dependent family member for subsistence. |
Beneficiary (Eg living below poverty line or senior citizens or students etc.) |
1. Age must between 18 years to 60 years. 2. Name must be entered in BPL list. 3. Must be head of household or main earning member of family. |
Benefits to the beneficiary |
On the death of the head of the family or the chief earning member of family between the age of 18 years and 60 years, a lump sum amount of Rs. 20,000 / – assistance to dependents. |
How to avail the scheme (Point-wise complete application process) |
1. Complete the prescribed application form. 2. Death certificate. 3. Samagra I.D. 4. Aadhaar Number 5. Mobile Number 6. Bank Pass Book 7. Age proof certificate 8. Must be a native of Madhya Pradesh
All the above documents are submitted to the Municipal Office and pension can be accepted as per eligibility or can also be obtained by applying in Public Service Guarantee. |

Benefits:
In the event of death of the head of the family or the chief earning member between 18 years and 60 years, the member of the dependent family gets a lump sum of Rs. 20,000 / - is supplied.
How To Apply
1. Complete the prescribed application form
2. Death certificate
3. Composite I.D.
4. Aadhaar Number
5. Mobile Number
6. Bank Pass Book
7. Age proof certificate
8. Must be a native of Madhya Pradesh
All the above mentioned documents are submitted to the District Panchayat in the rural area and the Municipality / Municipal Council in the urban area, as per eligibility, or pension can be obtained by applying in the Public Service Guarantee.
